The primary use of the facilities at Colorado Academy is to carry out the educational mission of the school. To the extent that space is available and the rental is approved, Colorado Academy welcomes outside organizations to use the facilities for purposes consistent with the school’s mission. Any group that uses Colorado Academy’s facilities is expected to conduct themselves in a professional manner, and follow the stated policies. Further, these gatherings shall not disrupt normal school functions, endanger the health or safety of any person present, or damage the school’s property or reputation.
Colorado Academy’s internal programs have priority use of all facilities. All individuals or groups wishing to rent space from CA must first submit this form for review by the Auxiliary Programs Office. If confirmed, the individual or group must assume responsibility for compliance with all policies and procedures stated in a Facility Use Agreement (Contract).

Use will be prioritized in the following order:
1. CA's school sponsored activities
2. Non-profit groups who are not charging fees
3. Non-profit groups who charge fees
4. Youth training, practices, and matches
5. Youth tournaments and performances with spectators
6. Adult recreational activities

RENTAL RATES (subject to change at any time)

* At least one CA staff member must be on-site for the duration of the rental; depending on the needs of each rental, additional staffing, spectator, and/or cleaning fees may be assessed.

IMPORTANT
- All rental requests must be reviewed and approved by the office of Auxiliary Programs, as evidenced by a signed Facility Use Agreement
- School sponsored programs have priority use of all facilities
- Colorado Academy administration reserves the right to approve or deny any rental requests

INSURANCE REQUIREMENTS
CERTIFICATE OF INSURANCE (COI) REQUIREMENTS

Upon approval of the requested rental, User shall furnish to CA, in a form satisfactory to CA, a certificate of insurance (“COI”) evidencing and maintain the following insurance policies and terms:
1. Commercial General Liability Insurance (public liability) with limits not less than one million dollars ($1,000,000) per occurrence and two million dollars ($2,000,000) annual aggregate,
2. Umbrella Liability to cover General Public Liability and Employer’s Liability in amounts not less than one million dollars ($1,000,000),
3. Sexual Abuse and Molestation Insurance with limits not less than one million dollars ($1,000,000) per occurrence and two million dollars ($2,000,000) annual aggregate,
4. Worker’s Compensation Insurance as required by law, and Employer’s Liability Insurance with limits not less than one hundred thousand dollars ($100,000) bodily injury by accident/each accident, one hundred thousand dollars ($100,000) bodily injury by disease/each employee, and five hundred thousand dollars ($500,000) bodily injury by disease/policy limit,
5. User shall maintain a Commercial Automobile liability policy with a combined single limit of not less than one million dollars ($1,000,000) to include owned, non-owned, leased, and hired car coverage,
6. Providing a waiver of subrogation against all Colorado Academy Parties (as further defined in section 13 below) for general liability, automobile, and workers’ compensation policies,
7. Listing Colorado Academy as Additional Insured on the Certificate of Insurance for general liability, automobile, umbrella and excess liability policies,
8. Including Primary and Noncontributory Coverage for general liability, umbrella, and excess liability policies, and
9. Acknowledging the insurer’s agreement to provide Colorado Academy no less than thirty (30) days’ written notice prior to any cancellation or material alteration of the insurance coverage.
I understand that any insurance obtained by CA is for CA’s own benefit and CA shall be under no obligation to obtain (and will not obtain) insurance for or on our behalf.
